At Burlingame Dental Studio, we’ve made your visit easier and more convenient by going completely paperless!

We do not provide downloadable or printable PDF forms. Instead, the process begins when you schedule an appointment with our office. Once your appointment has been confirmed, you’ll receive a secure link from our trusted service provider, Adit, to complete your online patient forms right from your phone, tablet, or computer.

This system helps us:

  • Keep your information secure and accurate
  • Reduce paper waste and streamline your check-in experience

If you’ve already scheduled an appointment but haven’t received your Adit forms, please check your email or text messages for the link. Still can’t find it? Contact our team, and we’ll be happy to resend it.

Print & Fill

If you’d prefer to complete your forms ahead of time, you can download and print them using the link below. Once filled out, simply bring the completed forms with you to your appointment. This option is ideal for patients who have not yet received the automated online forms or prefer to handle their paperwork manually.
